Working in Our Community

The Alta Healthcare District Board is seeking your input as it assesses the current healthcare needs of residents within the District.  You opinions are being sought, either because you have previously worked with the District or we are very interested in your thoughts and guidance.  Please take a few minutes to complete the below survey and return it by e-mail to ybotello@altahcd.com or by regular mail to Alta Healthcare District P.O.
